Periodontal disease is the most common condition in dogs and cats: it affects 80% of dogs over 3 years old and 70% of cats the same age. Despite these figures, dental hygiene remains the most neglected area of pet care. The consequences go far beyond bad breath: chronic periodontal infection releases bacteria into the bloodstream that can damage the heart, kidneys and liver.
How Periodontal Disease Develops
Dental plaque is a film of bacteria that continuously forms on teeth. If not removed mechanically, it mineralizes in 3-5 days to form tartar (dental calculus), a hard, porous structure that cannot be removed with a brush and requires ultrasonic scaling under anesthesia. Tartar harbors even more bacteria, which slide below the gum line causing gingivitis (reversible) and, if untreated, periodontitis (alveolar bone destruction, irreversible).
The process is silent: dogs and cats rarely show dental pain obviously until the disease is very advanced. This is why prevention and early detection are fundamental.
Tooth Brushing: The Most Effective Method
Daily brushing is the only proven method to effectively remove dental plaque before it mineralizes. There is no alternative of comparable efficacy.
Keys to successful brushing:
- Never use human toothpaste: it contains fluoride and xylitol, toxic to dogs and cats if swallowed. Veterinary toothpastes have attractive flavors (chicken, beef, mint) and are safe if ingested.
- Appropriately sized brush: finger brushes for small dogs and cats, long-handled brushes for large breeds. A 45° angle to the gum line is most effective.
- Habituate from puppyhood or immediately: habituation is gradual. First touch the muzzle, then the lips, then the teeth with a finger, and finally with the brush. The process can take weeks.
- The outer (vestibular) side is most important: the tongue helps clean the inner side. Focus on the upper carnassials (the largest and most tartar-prone teeth).
Alternatives to Brushing: Evidence and Reality
For owners who cannot brush their pet's teeth, there are alternatives that, while less effective, are better than nothing. The Veterinary Oral Health Council (VOHC) certifies products with demonstrated efficacy through clinical studies:
- Chlorhexidine gels and rinses: antiseptic that reduces bacterial load. Moderate efficacy. Can stain teeth with prolonged use.
- VOHC-certified dental treats (such as Oravet or CET Chews): help reduce plaque through mechanical action during chewing. Should be used daily.
- Prescription dental diets (Hill's t/d, Royal Canin Dental): kibble with special size and texture that generates cleaning friction when chewed. Not to be confused with regular commercial kibble labeled as "dental."
- Chew toys: only firm rubber ones (Kong, Nylabone) have some effect. Raw bones can cause dental fractures and are not routinely recommended.
Dental sprays, rinses without chlorhexidine or plain yogurt do not have sufficient evidence to be recommended as a primary method.
Professional Dental Cleaning: When and How
When tartar is already established, the only treatment is dental prophylaxis under general anesthesia. The procedure includes: ultrasonic scaling to remove supra- and subgingival tartar, polishing of tooth surfaces to delay new plaque formation, periodontal probing to detect pockets, intraoral dental radiographs (essential, as 60% of tooth structure is below the gum line) and extraction of non-viable teeth.
Risk-free anesthesia does not exist, but modern anesthetic protocols are very safe even in older animals. "Anesthesia-free" dental cleaning is not only ineffective (it does not allow working below the gum line) but can also be stressful and dangerous for the animal.
The frequency of professional prophylaxis depends on each animal's tartar formation rate: some need annual cleaning, others every 2-3 years with good home hygiene.
